Home Care Medical Records Secretary Salary: Grade 06 $11.80 - $15.35/hr (midpoint) F! acility: Cheyenne Regional Medical Center Department: Cheyenne Home Health Admin Schedule: Full-time Shift: Day shift Hours: 8-5 Job Details: High School graduate or equivalent required ROLE SUMMARY Works under the supervision of the Home Care Business Office Supervisor or Manager in Branch Office. As the primary receiver of all incoming telephone calls the medical records secretary is responsible for providing excellent customer service and direct calls to appropriate staff. Is responsible for the support and overall smooth functioning of home care medical records to include assembling new medical record charts and auditing discharged charts. These responsibilities include, but are not limited to: accurate and timely filling in medical records, distribution of mail and faxes and other general clerical duties.

In a laptop environment, this includes printing of documents as necessary for the clinical staff to include the Plan of Care, med sheets, and notes for submi! ssion to the MD. In the non laptop environment, this includes ! data entry of admission information, OASIS, generation of the Plan of Care (485), and med sheets. The MRS is responsible for the submission, tracking, and receiving of all physician orders. This person will enter admission data for MSWs, STs, and Pediatric PT and OTs into the Horizon Homecare system and print out resulting date.

Additional necessary to maintain the smooth operation of the HCS department. CORE RESPONSIBILITIES 1. Receives incoming calls to the department or unit and redirects accordingly. Takes messages as appropriate and delivers them promptly.

2. Manages clinical medical records creates new charts and audits discharged charts. 3. Medication profiles are generated within two working days of receipt in the office.

May assist in the process of notifying the physician of the interactions. Maintains current medication profile as applicable. 4. Plans of Care are completed within two working days of receipt in the office.

Ident! ifies errors or incomplete documentation, assists in correction of the problem. Admission data entered into system within 24 hours of receipt in the office. 5. Maintains control of timeliness of documents.

Plans of Care and physicians supplemental orders are signed and in medical record within thirty days. Maintains accurate control of physicians order tracking including submitting, tracking and re-issuing as necessary. Assists in billing/audit process by notifying the RC of late orders received. May assist in the delivery and pickup of the physician orders.

6. Provides PCM/designee with a recert list twice per month. Maintains accurate census. 7.

Takes minutes of Home Care services staff meeting. 8. Acquires a minimum of 5 continuing education units per year. SKILLS, KNOWLEDGE, AND ABILITIES Excellent verbal, written and interpersonal communication skills Excellent people skills Ability to maintain confidentiality regarding employee issues and HR! business matters.

Proficient with MS Office, Outlook and Inte! rnet Explorer Excellent organization skills and attention to detail Demonstrates an independent work initiative, sound judgment and attention to detail Ability to handle multiple tasks simultaneously MINIMUM REQUIREMENTS High school diploma or GED or equivalent education and experience One year of administrative support or general office experience PREFERRED QUALIFICATIONS College level course in computer or business office education. Medical terminology knowledge and medical record experience.
